We are currently seeking a highly skilled and compassionate Primary Care Provider / Nurse Practitioner with a focus on geriatrics, hospice, and palliative care to join our primary care team. Responsibilities:
- Provide high-quality primary care services to geriatric patients, focusing on promoting healthy aging and managing chronic conditions.
- Conduct thorough assessments, perform physical examinations, and order and interpret diagnostic tests.
- Develop and implement individualized care plans based on patient needs, including medication management, preventive screenings, and health promotion activities.
- Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure holistic and patient-centered care.
- Assist in coordinating and providing hospice and palliative care services, including pain management, symptom control, and emotional support for patients and their families.
- Educate patients and their families about disease management, treatment options, and end-of-life care decisions.
Requirements:
- Current and unrestricted Nurse Practitioner license in Utah.
- Minimum of 2 years of experience as a Nurse Practitioner, with a focus on geriatrics, hospice, or palliative care.
- Strong knowledge of geriatric healthcare, chronic disease management, and end-of-life care principles.
- Excellent clinical assessment and diagnostic skills.
- Ability to work collaboratively in an interdisciplinary team environment.
- Exceptional communication and interpersonal skills, with a compassionate and empathetic approach to patient care.
- Ability to travel in the Utah Area.
